Staff transitions: How to preserve and transmit knowledge, experience, and relationships

What to do in advance?
Turnover documentation - also be open to the new person having a new idea.
Documentation - start with links to existing docs, get new person to both research and add missing pieces.
Top 5 things documentation - Who do you contact? What resources are used/needed?
Most recent things you have done/worked on.
Video the person doing the work - new person transcribes the notes.
Interview person doing the work - new person transcribes the notes.
Interview peers and co-workers on what they think the person does.
Documentation - what was a particular decision made? Who made it and why? The why is more important than the what.
Use brief summaries of goals and accomplishments from appraisals to identify work.
Cross train as much as you can ahead of time so knowledge is shared.
Try to have two people on each project/task for shared knowledge.
Schedule time to do documentation, transition, etc. (as part of regular work or in advance is even better; make it part of annual goals).
How to manage the transition?
New person shadowing - then switch roles.
Shadowing for a period of time - then have new person in role with previous person available.
Meet with other people around the university you need to work with.
Pair people up to keep shared knowledge.
What can you do afterwards?
Can prior employee be available by email or other?

Re-evaluate the importance of what you do.
Re-evaulate if new skills may be appropriate.
Re-evaluate the processes used.
Issues
Takes a lot of time and effort.
Most important information; where to start - where to find the string to pull and why you do it this way,
Most important info; what access do you have, what groups are you a part of, etc.
Keep list of what you actually do and work with HR.
Frequency and when you do work and partners you work with.
Provide context - how does this task/your role fit in to the dept/school/university goals? 
Tools
Capture info into a wiki or other repository.
Slack - or other archiving repositories
